Start! On-Demand Reporting Center Release and Indemnification

The On-Demand Reporting Center is a tool that can help me track the activities of my company’s employees who’ve registered and are using the MyStart! Online tool. I have been given access to certain personal information from those who have opted in to share it with our company. I may also see certain aggregate information from all participants.

As this is personal information granted to be viewed by only me and/or other company delegates,  I hereby expressly assume all risks, including privacy issues, arising in any way out of my participation in the MyStart! Online Tool On-Demand Reporting Center.

It is my responsibility to share information from this report only with appropriate company representatives.

I agree, for myself, my heirs, executors and administrators, to not sue and to release, indemnify and hold harmless, the American Heart Association, Inc., its affiliates, officers, directors, volunteers and employees, and all sponsoring businesses and organizations and their agents and employees, from any and all liability, claims, demands, and causes of action whatsoever, arising out of my participation in using this data and sharing information ONLY with appropriate company representatives. - whether it results from the negligence of any of the above or from any other cause.

This release and indemnification agreement shall be as broad and inclusive as is permitted by the State or Province in which the event is conducted. If any portion of it is held invalid, the balance shall continue in full force and effect.
